Only spreadsheet files, either comma delimited (.csv) or Excel files (.xls, .xlsx) are accepted.

Please ensure:

• The first row of the file contains a row for headers.
• Note: The system will choose the first 5 fields to show as the fields for the master list.
• The order in which the fields show up the editing page is determined by the order of the columns in the file.
